Insurance:

Dr. Vallese is a PPO provider. Please let us know your current insurance information when making your appointment.

 

Our front desk coordinator will assist you with your insurance paperwork to make sure that your claim is processed to completion.  

Each patient is responsible for any balance remaining after insurance pays on a claim.  If you do not have dental insurance, you are responsible for payment in full at the time of the procedure in our office.

 

It is the patient's responsibility to provide us with any new information pertaining to your contact information or insurance policy. 

Patients are responsible for any balance payment that insurance will not cover, either at the time of treatment, or through a pre-arranged financial agreement.  If for unforeseen reasons your insurance carrier denies your claim, you are responsible for the payment in full.

We accept all major credit cards, cash, and care credit.

 

Financing Options:

We are pleased to offer a range of in-house payment plans.  Should your estimated balance be in excess of what you can afford, please see our front desk coordinator to discuss options for a financial agreement.
