Insurance Policy
Dr. Vallese is a PPO provider. Please provide your current insurance information when scheduling your appointment. Our front desk coordinator will assist you with your insurance paperwork to ensure your claim is processed to completion.
Each patient is responsible for any balance remaining after insurance pays on a claim. If you do not have dental insurance, you are responsible for payment in full at the time of the procedure in our office. It is the patient's responsibility to provide us with any new information pertaining to their contact information or insurance policy. Patients are responsible for any balance payment that insurance will not cover, either at the time of treatment or through a pre-arranged financial agreement.
If, for unforeseen reasons, your insurance carrier denies your claim, you are responsible for the full payment.
Financial Policy
For your convenience, we accept payments made with cash, debit cards, Visa, MasterCard, Discover, and American Express. We also offer financing through CareCredit®.

Cancellations
Kindly provide at least 48 hours notice for any appointment that needs to be rescheduled. Appointments cancelled in less than 24 hours may be subject to a cancellation charge.
